Health Assessment

Part of staying healthy and managing diabetes is having regular health checks and routine monitoring of complications. Health checks and monitoring include the following tests among other things:
- Eye screening (with an eye specialist or eye photography) This will be done within the first year of diagnosis for people with Type 2 diabetes and usually after 5 years in people with Type 1
- Foot check
- Blood lipids (e.g. Cholesterol)
- Weight and BMI (body mass index)
- Kidney function
- Diabetes management
Make the most of your next visit to your doctor by being as organized as possible by creating a checklist of all the things you want to discuss about your self-care plan. By doing so, it can help ensure that you are on the right track and in complete control of your diabetes.
Here is a checklist of helpful information regarding your current health that you should bring with you to your next doctor visit:
- Your latest blood sugar results and trends
- How you are feeling; specific issues with eyes, feet, other body parts
- How you are managing your meal plan, weight, stress and exercise
- Symptoms of any illness you might be experiencing
- Any other general questions or concerns you want to discuss
